modem return policy

  1. When a customer deactivates DSL service, the customer is responsible for returning the modem to FairPoint Communications (by mail or hand delivery) within 15 business days. Customers who fail to return the modem within 15 days of deactivation will be billed $99.00 for the cost of the modem.

  2. All returned modems MUST be accompanied by the following information:
    • Customer Name
    • Customer Address
    • Customer Telephone Number
    • FairPoint Account Number
    • Date Service Canceled
    • Date Modem Returned
    • FairPoint Email Address

  3. Customers can return modems in person to any of the following FairPoint business office locations during regular business hours:
