The monthly charges do not include any equipment charges, taxes, fees,
other surcharges, or long distance charges beyond any allotted minutes.

Additional minutes of long distance use beyond the allotted minutes of your
plan are billed at 10 cents per minute. If you no longer use Northland Long
Distance as your long distance carrier, the bundled offering will no longer
apply, and local service and optional features will be billed at normal
tariffed rates. This offer cannot be combined with other discounts or promotions
unless otherwise indicated. Customers must be authorized to make account changes
to all accounts. Customers with a security deposit, credit limit, non-basic block
